Parent Opt-Out Response Form

Dear Parent/Guardian,

Alder Creek Middle School is offering a variety of support groups to interested students. These groups are designed to promote positive self-esteem and help students learn about several topics, including dealing with grief and loss, saying "no" to drugs, coping with a loved one who abuses chemicals, dealing with divorce, conflict resolution skills, anger management techniques, and empowerment, etc.

Sessions are optional and your child may volunteer or be referred to participate in one or more depending on need and interest. Groups generally meet once a week, during one class period on a rotating basis, for six to eight weeks. If you approve of your child participating in this program, no response is necessary. If you DO NOT want your child to take part, please print, sign and return the form below to the Main Office.

If you have any questions or want to refer your child for one of these groups, please feel free to contact either of the counselors, Stephanie Atkinson 503-353-5718 (students last names A-K) or Brian Monson 503-353-5717 (students last names L-Z).
